I am over 18 years of age or have received the necessary permissions from my legal guardian.

I am not on any medication, drugs or undergoing any course of treatment, nor influenced by alcohol that could affect my ability to engage in parachute training and/or carry out parachute descent.

For myself, my heirs, executors and successors INDEMNIFY AND FOREVER HOLD HARMLESS Skydive Tandem Johannesburg, Angels Way Trust and Trustees, Warbirds, Skydive Johannesburg, Skydive The Farm and associated companies and personal agents and contractors against all claims, costs, damages and liabilities of any kind whatsoever (which includes acts or omissions whether due to negligence or otherwise), in respect of any physical or mental injury or damage to property of any degree or nature whatsoever caused by myself, my heir, executors or successors for anyone whomsoever resulting directly or indirectly from my parachute training and/or parachute descent.

I declare that I am of sound and sober senses and the above information is correct. Parachuting and skydiving is of a potentially hazardous nature and by participating there in, persons do put themselves at risk of either injury or death. I hereby indemnify the organizers, staff, Dropzone operator. Skydive Tandem Johannesburg, Skydive Johannesburg, Skydive The Farm ., Dropzone Staff, Warbirds aircraft operators –Johan Lok, Strong Enterprises, United Parachute Technologies, against any civil or criminal action directly or indirectly resulting from injury or fatality which has anything to do with the disciplines, operations or procedures, connected to or relevant to the sport parachuting. I am fully aware of the limitations of any drop zone and declare that registering to jump is done at my own free will, accepting the conditions laid down by the organizers. I there f or also undertake to abide by the basic safety regulations – Manual of Procedures, as laid down by PASA / ADZO as well as the organizers. (Parachute Association South Africa / Association Drop Zone Operators).

I understand that due to extreme conditions associated with filming and skydiving, aerial footage can not be guaranteed. Tandem Skydive and Tandem camera Sold separately.

I have read and understand the above and agreed the above are the terms and conditions upon which I undertake the parachute training and/or parachute descent and/or passenger in aircraft.
